I understand that aesthetic treatments, including but not limited to injectable treatments, liposuction procedures, laser treatments, and related services carry inherent risks including bruising, swelling, infection, asymmetry, and in rare cases, more serious complications.

I confirm that I have disclosed all relevant medical history, medications, and conditions to the best of my knowledge, and I understand that withholding information may affect the safety of my treatment.

I understand that results are not guaranteed and may vary. I have been given the opportunity to ask questions and have had them answered to my satisfaction.

Maison Aesthetics maintains strict confidentiality of all patient health information in accordance with HIPAA regulations. Your information will only be shared with treating providers and will not be sold or disclosed to third parties without your explicit consent.

Photo / Media Release

You may optionally consent to before/after photographs for clinical documentation purposes. These images are stored securely and used solely for tracking your progress unless you explicitly authorize other use.
